Notice of Privacy Practices
The Hoag Hospital Foundation (“Hoag”) adheres to the requirements outlined by the Health Insurance Portability and Accountability Act ("HIPAA"), which ensures the security and privacy of an individual's medical records and promotes privacy and trust between patients and their healthcare providers.
The Hoag Hospital Foundation will make available a current Notice of Privacy Practices to each individual that presents at Hoag for healthcare services. The Notice describes how Hoag uses and shares patient personal health information.
